Patient Information

Arriving for Your 1st Appointment

If you are a new patient, please arrive 15 minutes prior to your appointment time to complete your medical/dental history form we require for your file. If you wish to fill out the form prior to your first appointment here is a link where you can download it: Patient Registration Form

Parents and/or guardians must accompany all patients under the age of 18 to their appointments.

Please Bring to First Visit

  • If your previous dentist has taken x-rays within the past 6 months you may request to have them forwarded to our office in advance of your appointment or bring them with you
  • A list of any current prescription medications you are taking. Please alert us if you require premedication before treatment or if you have any allergies
  • If you have a public or private german insurance (e.g. TK, AOK, Barmer, GEK, etc.) please bring your insurance card (Krankenkassen Karte) with you

Generally we do not request dental treatment records to be forwarded to us.

Arriving Late
When a patient arrives late for an appointment, it may or may not be possible to start or complete the treatment in the remaining reserved amount of time. To provide you with proper professional care, it may be necessary to reschedule your appointment, or book an additional appointment to complete treatment.

Same Day Urgent Care
Our office is pleased to provide prompt same day or walk in care as needed by returning or new patients. Urgent same day care is available for toothaches and acute dental pain. No referral is required. We recommend you to call us or write us an email before you come in for emergency dental treatment. Urgent care is generally managed either same day or next day depending on patient circumstances and requests.

We will always diagnose and treat you based on your actual health needs without regard to the limitations imposed by your health insurance coverage. To treat you based on only your coverage would not be ethical. Our promise to you is to provide you with the finest care, at the most reasonable cost, regardless of insurance coverage.
However if you wish dental care according to your health insurance specifications please let us know this in advance.
